As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate as a team member in WCRD. I understand and fully acknowledge that certain physical hazards may be encountered in Buddy Ball during practice and play of this sport. I am willing to assume these risks on behalf of my child. I hereby certify that my child is/are healthy and has no physical or mental disabilities or infirmities that would restrict participation in these activities, except as listed on his/her medical release form to be handed in and kept by his/her manager. I hereby waive any claim for damages against Wayne County Recreation, Managers, coaches, agents, sponsors, or representatives on behalf of the participants in case of personal injury.

About Buddy Ball:

The Buddy Ball Division was established as a program for children with special needs.

Gives everyone a chance to play, regardless of disability.

Players are matched with volunteer “buddies”. Or “Teams”

Games will be held during the week:

Location: Babes Mighty Mite Fields/Ben Park

Practices will be left up to the coach

Please bring your own gloves and water bottles.

Games will be played against other WCRD teams.
